Program Overview
Our Respiratory Therapists will work closely with your team to identify and support residents who can benefit from respiratory care. To begin, we require EMR access to review resident diagnoses in the Minimum Data Set (MDS). This allows us to identify individuals with conditions such as, but not limited to:
After reviewing diagnoses, we will obtain face-sheets and secure pre-authorizations to confirm coverage. On the first day, our team will screen and evaluate all residents to identify additional candidates for the program. Once established, one Respiratory Therapist will be on-site daily for eight hours per 15 residents on the caseload.
Note: We do not bill the facility for long-term care (LTC) residents.

Billing & Benefits
For Medicare Part A residents, we bill the facility $1 per treatable minute, up to 30-minutes per resident. We do not bill Part A while the facility is also billing under Part A.
By providing 15 minutes of respiratory therapy for seven consecutive days, your MDS Coordinator can capture this service during the 7th or 8th day look-back period. This can increase your PDPM rate by approximately $100 per day, yielding a net gain of about $85 per day after a $15 carve-out. This translates to potential additional Medicare revenue of $2,550 per month per resident.
In Mississippi, a casemix state, our services for LTC residents can boost Medicaid rates. For every 0.01 increase in a resident’s casemix index, the Medicaid rate rises by $0.70. Residents receiving respiratory therapy often achieve a high nursing score (typically around 1.45, though this varies by state). For example, increasing a resident’s casemix index from 0.85 to 1.45 through therapy can significantly enhance facility revenue without additional nursing care. Together, we outline a path forward that’s realistic, strategic, and tailored to your specific needs.
Reducing Hospital Readmissions
Our program is designed to lower hospital readmission rates through proactive respiratory care, such as chest percussion therapy (CPT), which promotes pulmonary hygiene and maintains clear airways. This daily care reduces the risk of respiratory complications, much like routine dental hygiene prevents oral health issues.
As a former Administrator, I observed that residents recently discharged from hospitals often faced rapid readmissions due to inconsistent CPT. Hospitals typically provide CPT three to four times daily, a frequency difficult to maintain in nursing homes.
Our daily CPT program bridges this gap, significantly reducing readmission risks and supporting Value-Based Purchasing outcomes. Facilities with lower Return to Hospital (RTH) rates may see minimal repayments or even positive financial returns.
